Health Record Form and Immunization Records
Transcript or Report Card for the past 3 academic years
School Recommendation
Please note your application will not be considered until all supporting documents are submitted.
Admissions screening
Once the application is received the admissions office will provide a date for the admissions screening.
Admissions decision issued
Notification sent to the applicant regarding the decision.
